Know Where Your CLIA Communications Come From.

CLIA-certified laboratories in several states have received solicitation documents from a third-party that look like official CLIA bills or invoices and may be mistaken for official CLIA communications.  These documents are not issued by CMS. Official CLIA communications only come from CMS or your State Survey Agency.

You can check your laboratory's certification and survey status for free, anytime, through the QCOR CLIA Laboratory Lookup.

Remember, CLIA Is Now Paperless!

CMS no longer mails paper certificates or fee coupons and no longer accepts paper checks to pay your certification or survey fees. Keep a valid email address on file to receive these certificates, fee coupons, and other CMS communications electronically.

Need help?

  • Review the latest Quality and Safety Special Alert for more details.
  • If you receive a solicitation, invoice, or other correspondence that appears to be from CMS or a government source, review it carefully. Check the sender, purpose, and any requested fees or payment instructions.
  • Contact your State Agency or the CMS CLIA program at  LabExcellence@cms.hhs.gov if you have questions.

Use the Certification Quick Start Guide to help you complete the CMS-116 application.

Laboratories must pay their CLIA certification fee online though pay.gov, a secure platform hosted by the U.S. Treasury Department.
